Fairlawn Primary Care, Inc. is pleased to announce that our practice received High Scores for
Quality Care Measures for patients seen in January and February 2018.
75% of patients age 65 and older completed a fall risk screening in the past 12 months
76.8 % of female patients age 50-74 obtained a breast cancer screening in the past 27 months
88.2% of patients age 65 and older obtained a pneumonia vaccine
95.3% of diabetic patients completed a microalbumin lab test in the past year
The rate of repeat inpatient hospitalizations decreased to 0%
100% of patients surveyed in February and March 2018 responded “Yes” to:
Do you feel you can schedule an appointment or contact your provider easily?
Do you feel that your provider listens to you and answers your questions clearly?
Do you feel respected by the staff?
Do you feel up-to-date on test results and specialist referrals?
Do you feel you understand your medications?
Do you feel that all your health care needs are addressed?
Do you feel you receive the support you need to manage your health well?
